The SGMP membership application is available as a PDF (Adobe Acrobat) file. You may download this file, then print it locally from your computer. You need to have Acrobat Reader to view and print this file. This software is available free of charge. There is also a Microsoft Word version if you do not have Adobe Acrobat Reader, but we would prefer you use the PDF file.

Complete the printed application. Planners AND CONTRACT PLANNERS must also complete and return the planner profile with the application. If you wish to affiliate with chapter, MAIL your ORIGINAL SIGNED AND DATED application, along with your membership fee payable to SGMP, to the membership contact listed for the chapter you wish to affiliate with.
