Complete the printed application. If you wish to affiliate with a chapter, MAIL your ORIGINAL SIGNED AND DATED application, along with your membership fee payable to SGMP, to our headquarters address below. There is a $40.00 return check fee.

Note: Supplier applications must be submitted with a completed planner application or they will be returned to the applicant. Suppliers are required to join the same chapter as their planner match.

Membership Name Change Policy

Changes in name, address, and telephone number must be submitted in writing to the SGMP Membership Office. Print or download the Name Change Form below and complete. Please print or type! Be sure to sign your form and provide payment information! It will NOT be processed unless it is signed and payment information provided. Note: Effective May 1, 2008 there will be a $25.00 fee assessed for submitting a name change for all members.
